This year, the West Virginia Department of Education and Governor Joe Manchin, III are encouraging all entering kindergarteners to obtain a physical examination from a health care provider before registering for school. In addition, parents or guardians must bring proof of residency, an official birth certificate with a raised seal, the child’s immunization records, and the child’s social security card.

If you need assistance in obtaining a physical examination for your child, you may call the Putnam County DHHR at 304-757-7843, ext. 100, for assistance.
